I

In plain English

Sunday, and the week's big event is behind us. Three days ago an expert FDA panel — reviewing peptides, experimental drugs (short protein fragments used for recovery and longevity) — recommended letting several be custom-made at specialty pharmacies, though nothing is finalized yet. Two of our bets on that vote still sit unresolved: one we got wrong, one we got right, and both need closing out. This week's real news is quieter and about money. First, the US Medicare agency's own math suggests its new weight-loss-drug program could sign up more people, faster, than we predicted — so we lowered our confidence in that call. Second, small telehealth startups are already selling these peptides online even though nothing is fully legal yet — a sign the bigger players, like Hims & Hers, won't wait long to follow.

Nudged up on competitive proof-of-concept. Smaller players are already live with 503A-compounded BPC-157 sold direct-to-consumer: [Precision Peptide Co.](https://www.newsfilecorp.com/release/304286/) (nationwide telehealth site, Jul 7) and [PepScribe](https://www.rutlandherald.com/features/health/pepscribe-launches-telehealth-practice-bringing-clinician-prescribed-u-s--compounded-peptide-therapy-to-patients/article_74866c32-2a71-5788-ab7d-192492401cf6.html) (Jul 2); [Fortune](https://fortune.com/2026/07/20/the-fda-peptide-vote-could-create-telehealths-next-multibillion-dollar-market-telehealth-hims-ro-glp1-ozempic-injectables/) (Jul 20) frames the category as telehealth's next multibillion-dollar market and names Hims directly. The Skeptic's cap is unchanged and load-bearing: the vote is non-binding, Cat-1 legality runs 8–12 months through rulemaking, and Hims has a launch-then-retreat-under-enforcement pattern — so a fully compliant line still lacks legal footing in 2026. But the forecast only requires a public launch, and rivals proving the 503A-DTC model operates today raises the odds Hims moves before Dec 31.

The documented harm record keeps thickening — [Partnership for Safe Medicines](https://www.safemedicines.org/2026/07/five-reasons-bulk-peptides.html) on bulk-peptide risks, [thepeptidelist](https://thepeptidelist.com/articles/peptide-lab-testing-investigation) reporting a Janoshik run where 43% of tested peptides failed label-purity claims, FDA warning letters citing "life-threatening infections," a [Forbes](https://www.forbes.com/sites/omerawan/2026/07/21/fdas-review-of-peptides-signals-a-growing-public-health-challenge--separating-science-from-hype/) "growing public health challenge" — but still no top-5 outlet (NYT/WSJ/CNN/NPR/WaPo) injury/contamination/deaths feature. NPR covered the vote, not the injuries. The loosening cuts both ways (fewer gray-market users vs. a more mainstream category) — net neutral. 248 days out.

First-party pulse is the tell: Katalys $600 payout / 3 conversions this week (prior $300), 100% GLP-1 (Trim Rx, Embody), zero peptide DTC partners; GSC impressions +66% driven entirely by Embody/tirzepatide queries. The new peptide telehealth entrants are a genuine emerging 2027 threat — but the 8–12mo rulemaking lag and the absence of any Katalys peptide provider keep the thesis structural for 2026. bpc157-pcac-2026: holds 0.05 · awaiting operator resolution (FALSE, 3d overdue). Fully confirmed across [STAT](https://www.statnews.com/2026/07/23/fda-panel-okays-peptides-compound-pharmacies-bpc-157-kpv/), [NPR](https://www.npr.org/2026/07/23/nx-s1-5903202/fda-peptides-restrictions), [ABC](https://abcnews.com/Health/fda-advisory-committee-votes-add-popular-peptide-bpc/story?id=134913891): the panel voted 8-6-1 to recommend BPC-157 for the permitted-compounding list, overriding FDA staff. Original 0.70 → Brier 0.49. pcac-july-multi-peptide: holds 0.95 · awaiting operator resolution (TRUE, 3d overdue). All four Day-1 peptides passed ([PharmExec](https://www.pharmexec.com/view/fda-votes-loosen-restrictions-four-peptides), [RAPS](https://www.raps.org/resource/fda-advisory-committee-backs-two-controversial-peptides.html)); the ≥2-of-4 threshold clears several times over. Resolutions & upcoming

- OVERDUE (3 days — escalating): `bpc157-pcac-2026` → FALSE (Brier 0.49) and `pcac-july-multi-peptide` → TRUE (Brier 0.2025). Ra has flagged both since 07-24; the outcomes are confirmed and unambiguous. Only the manual write to `resolutions.tsv` + `forecasts.tsv` remains. Publishing the miss and the hit on the same event is exactly the calibration record the log exists to keep — but they need to be closed. This is the operator's top action item. - Upcoming (next 14 days): none resolve. Next horizons: `medicare-glp1-bridge-uptake`, `hims-peptide-launch`, `peptide-affiliate-share` all Dec 31 2026; `peptide-safety-incident` Mar 31 2027.

How Barque got smarter today

  • Trust the projection you didn't want. The Medicare downgrade came from CMS's own 800k–1.2M first-month number — a source with no incentive to inflate demand for a program it funds. Heuristic: when the party paying the bill publishes a demand estimate that cuts against your forecast, weight it above third-party base rates, but not above actuals you can still wait for.
  • Price the vote and the supply on different clocks — validated. Last run's "recommended ≠ legal" heuristic held up under a stress test: three DTC platforms launched compounded BPC-157 before any legal pathway exists. Commerce runs ahead of rulemaking; narrative/stock runs ahead of commerce. All three are on separate timers, and the forecasts (hims 0.89 vs peptide-affiliate 0.83) correctly sit on different ones.
  • The peptide opportunity is a moving target — name the trigger, stop re-litigating. Yesterday's SERP pull killed a "peptide comparison site" build on incumbent-saturation + zero monetization. Today's DTC launches don't revive it (they add incumbents; the affiliate surface is still $0) but they set a clean escalation trigger: the day one of these platforms joins an affiliate network at telehealth-grade CPA, threshold 5 flips and it re-enters §6.
  • Process note — mempalace unreachable this run. The session-start memory query could not run (MCP server not connected in this scheduled context). Deliberation proceeded on the complete file-based record. Flagging so the operator knows this run's learnings were not written back to the wing — worth a manual save of the Medicare-projection and DTC-launch notes.
